By working closely with each individual, their family, and/or relevant professionals, we create truly bespoke care and support plans. This collaborative approach allows us to provide high-quality, person-centred support for people with learning disabilities and/or autism of all ages with a wide range of care needs and aspirations.

Children & Young People

We offer nurturing, structured support that helps children and young people with disabilities and/or autism build confidence, develop life skills, and access meaningful opportunities at home and in the community.

Adults

Our support empowers adults with learning disabilities and/or autism to live independently, make their own choices, and enjoy a fulfilling daily life shaped around their individual goals and preferences.

Later Life

We provide gentle, dignified support for older adults with learning disabilities and/or autism, helping them stay safe, connected, and independent for as long as possible while maintaining a high quality of life.

Our dementia support focuses on comfort, reassurance, and familiarity, helping individuals with learning disabilities and/or autism to stay engaged and safe while preserving dignity and identity at every stage.

Autism & Emotional Regulation (Age 9)

Background: “Sam” is a 9-year-old boy with autism and sensory processing differences. School attendance had reduced significantly due to anxiety and emotional dysregulation. His parents were exhausted and fearful of permanent exclusion.

Challenges: School refusal, distress behaviours at home, sensory overload in community settings, parent burnout.

The Engage Approach: Engage introduced a small, consistent team trained in Positive Behaviour and Active Support. Engage also composed and implemented a fully person centred care plan including a full communication (PBS) and sensory profiling support section. We conducted a functional behaviour assessment, developed a predictable after and outside of school structure, Introduced graded community exposure, supported parents with regulation strategies, liaised with school and other professionals to ensure consistency.

Outcome: Within 12 weeks school attendance improved, emotional incidents reduced, parents reported improved confidence, Sam re-engaged with a community activity.

Benefit of the Engage Model: Consistency, structured planning, and family-inclusive support prevented escalation and preserved placement.

Complex Needs & Transition to Secondary School (Age 14)

Background: “Lily” has a moderate learning disability and epilepsy and is currently supported in a main stream school. Transition to secondary school created anxiety, regression in independence skills, and increased safeguarding concerns.

Challenges: Reduced independence, high parental anxiety, health monitoring complexity, social isolation.

The Engage Approach: We implemented a transition support plan, travel training and support programme, structured independence-building goals, multi-agency communication with school and health teams.

Outcome: Lily now travels short distances with support led by Lily, self-care skills improved, parent confidence increased, emergency health incidents reduced.

Benefit of the Engage Model: Holistic transition planning prevented dependency increase and promoted gradual independence.

Autism & Behaviours that Challenge (Age 27)

Background: “Daniel” had experienced multiple placement breakdowns due to behaviours that challenge. Previous services relied on reactive strategies.

Challenges: High staff turnover, escalation patterns, community exclusion, low self-esteem.

The Engage Approach: We established a consistent, specialist-trained team, Implemented Positive Behaviour Support, analysed behavioural triggers, developed structured daily routines, introduced meaningful community interaction so Daniel felt his life had purpose and meaning.

Outcome: Reduction in incidents, no placement breakdown, increased community engagement, improved emotional regulation, Improved self-belief and self-value.

Benefit of the Engage Model: Understanding behaviour as communication reduced crisis-driven responses.

Mental Health & Learning Disability (Age 38)

Background: “Caroline” lives with a mild/moderate learning disability and complex trauma history. Periods of mental health crisis led to hospital admissions.

Challenges: Emotional dysregulation, medication instability, social withdrawal, safeguarding concerns.

The Engage Approach: We implemented trauma-informed care, collaborative crisis planning, structured wellbeing routines, close liaison with other professionals including the community mental health team.

Outcome: Reduced hospital admissions, increased daily structure, improved engagement in meaningful activities, stronger multi-agency coordination.

Benefit of the Engage Model: Proactive, preventative support reduced crisis escalation and hospital reliance.

Early Dementia & Social Isolation (Age 76)

Background: “George” has a moderate learning disability and was recently diagnosed with early dementia. George and his friend live with minimal support in the local community. George was increasingly relying on his friend who also has a learning disability. This resulted in George staying at home more and withdrawing from social interaction.

Challenges: Memory deterioration, reduced confidence outdoors, loneliness, anxiety around his relationship with his best friend

The Engage Approach: We introduced a consistent companion support worker, structured cognitive stimulation activities, supported interaction into a community group, provided reassurance and communication between George and his best friend. Relationship-led care reduced isolation and preserved dignity.

Outcome: Increased community engagement, stabilised mood, improved daily routine, reduced stress both with his best friend and within the home.

Complex Physical Needs & Loss of Confidence (Age 79)

Background: “Fred” has a moderate learning disability and downs syndrome he recently experienced reduced mobility following hospital discharge. Confidence declined and risk of readmission was high.

Challenges: Falls risk, loss of independence, low mood, Increasing reliance on his friend as his care provider was at risk of breaking down the relationship.

The Engage Approach: We developed a graded independence plan, coordinated with professionals, including occupational therapist, district nurses and providers of his in-home direct care, encouraged both confidence and strength-building daily tasks, focused on empowerment rather than dependency.

Outcome: No hospital readmissions, improved mobility confidence, increased participation in daily tasks, emotional wellbeing improved, relationship with best friend returned to one of companionship not care.

Benefit of the Engage Model: Holistic reablement approach preserved independence and reduced long-term care dependency.

We support adults and young people with learning disabilities and/or autism and any associated additional needs they may have, including complex health conditions, and age-related challenges across West Norfolk.

We design and implement high-quality, flexible support plans that place each individual at the heart of everything we do. Whether it's providing a bit of extra help with daily routines, supporting engagement in community activities, or delivering a comprehensive package of care, our services are thoughtfully tailored to meet each person's unique needs, preferences, and goals.

How do I pay for support?

You can pay privately for our services, however many of our service users use local authority
funding through the Norfolk County Council, (through Day services for Adults) or Short Breaks,
(Funding for children under 18) or any Norfolk County Council personal budget. If you are unsure of where to start, feel free to give us a call on 01553 401200, and we can guide you through all your options.
